As a part of the Momentum Health4Me program, you can now get free, personalized coaching from a doctor via the app.
Download the program from the Google Play store and register on the app. Once you are registered, you can access the coaching and other services available on the app.
You will be able to see the available benefits for you and your child and decide which ones are right for you.
In addition, you can see your personal information such as your medical history and current medications.
You can also access your health benefits and membership card on the website.
If you are a member of the Momentum Health4Me program, you can access the services on the app.
